Health insurance premiums 2027: What is known before the announcement

Ahead of the 2027 premium round, the official series shows how sharply the burden has risen since 2022. The official figure for 2027 is still pending.

The health insurance premiums for 2027 are expected to be announced in the coming days. As of the editorial deadline, no specific nationwide increase had been officially confirmed. That is more than a formality: the Federal Office of Public Health (FOPH) is the body that reviews and approves the tariffs submitted by insurers. Until then, percentage figures remain forecasts – even when they come from industry circles.

Since 2022, the median premium has risen by just over 25 percent

The more recent FOPH series shows how sharply the burden has risen. The median monthly premium across all age groups rose from CHF 313.90 in 2022 to CHF 393.30 in 2026. That is a nominal increase of 25.3 percent within four years.

YearMedian monthly premium
2022CHF 313.90
2023CHF 330.80
2024CHF 359.50
2025CHF 378.70
2026CHF 393.30

FOPH media releases cited increases of 8.7, 6.0 and 4.4 percent for the 2024, 2025 and 2026 premium rounds. The rate has thus become smaller of late, while the premium level continued to rise. For 2025, the current FOPH fact sheet shows an effective increase of 5.7 percent in retrospect. The difference arises partly because insured people can switch insurer, deductible or model. Announced and actually paid average premiums are therefore not identical in every year.

The median premium is a weighted average across the different premium constellations. The personal bill depends on age, place of residence, deductible, insurance model and accident cover, among other things. The nationwide average shows the direction, but it does not replace an individual quote.

Why premiums and costs are not the same number

Premiums must cover the expected costs of mandatory health care insurance. For a specific approval, however, the FOPH takes into account more than a single mid-year cost rate. Reserves, risk equalisation, cantonal differences, tariffs and the composition of the insured pool also play a role.

Structural cost drivers include new and sometimes expensive treatment options, population ageing, rising demand for health services, and tariff changes in outpatient and inpatient care. At the same time, cost-cutting measures, cheaper medicines or the reserve position of individual insurers can influence the required increase. A personal premium can therefore rise much more or much less than the national average.

After the announcement, the personal quote is what counts

What counts is the premium notice for your place of residence, age group, model, deductible and accident cover. After that, comparing on Priminfo.ch is worthwhile. The FOPH calculator is the only official premium calculator of the federal government and compares approved tariffs anonymously, independently and without advertising.

Anyone who works at least eight hours a week for the same employer is generally also covered against non-occupational accidents through that employer. With the appropriate proof, accident cover at the health insurer can be suspended; the premium falls. In case of doubt, personal cover should be clarified with employer and insurer before any change.

With HMO, family doctor and telemedicine models, the rules of the individual product are decisive. A lower premium is only worth it if the required first point of contact fits everyday life. The deductible is not a pure savings lever either: a high deductible lowers the premium but increases the financial risk in the event of unexpected treatment. It is above all bearable when sufficient reserves are in place.

Families and households on tight budgets should also check the cantonal premium subsidy. Procedures and deadlines differ from canton to canton. Supplementary insurance is legally separate from basic insurance. Basic insurance can be switched without hastily cancelling an existing supplementary policy.

The average is not your personal bill

Once announced, the median premium will show how the burden is developing across Switzerland. For an individual household, however, it does not determine the amount on the bill. Place of residence, age, insurer, model, deductible and accident cover determine the actual premium.

That is why the most important part of the premium round only begins after the official publication: check your personal quote, compare on Priminfo, and only then decide whether switching insurer, model or deductible is worth it.
